Addressing Social Determinants With Action To Improve Health

With the shift to value-based care and incentives attached to quality outcomes, providers are looking beyond medical data to create a more comprehensive patient view that also identifies and considers social determinants of health that studies show have a significant impact on the patient’s health, risk for complications, readmission rate and access to care.
